# Retrieval quality audit agenda

## 1. Sample review

Review answers across known-answer, stale-source, conflicting-source, permission-denied, and unsupported-claim questions. Inspect the answer, source citations, permission trace, and reviewer correction.

## 2. Failure classification

Classify each failure as source freshness, source authority, chunking, ranking, permission filtering, prompt boundary, or reviewer workflow. Do not leave failures under a generic "model quality" label.

## 3. Remediation ownership

Assign each remediation to a source owner, platform owner, policy owner, product owner, or operations owner. Record the release gate that proves the remediation worked.
