- Sample known-answer, stale-source, conflicting-source, permission-denied, and unsupported-claim questions.
- Score source precision, source recall, freshness, answer boundary behavior, and reviewer correction severity.
- Assign remediation to source owners, platform owners, policy owners, or prompt owners.
- Re-run the sample set before every retrieval, model, prompt, or source-ingestion release.

## Decision standard

Retrieval is production-ready only when source ownership, permission behavior, audit cadence, and remediation ownership are visible. A high answer-quality score is not enough if citations cannot be inspected.
