## What it includes

- A postmortem map for detection gaps, routing delay, evidence quality, rollback decisions, and knowledge updates.

## How to use it

Apply the signal map to the services where incidents create customer-visible damage. During incidents, use AI-prepared summaries only when the source trace is visible and owner routing is explicit. After incidents, update the routing matrix and knowledge sources.
